The entry problem
Your PCP will talk about menopause, but won't say the word perimenopause even if they suspect it. They call it poor sleep hygiene, stress, or anxiety, and route you to behavioral health. Escalating to OB/GYN helps, but even then some won't say it either. If you're already in full menopause, you usually clear this stage; your problem starts later, at dosing.
The redirect trap
At a majority of places this is unwritten policy: instead of HRT, you get redirected, and it happens in both perimenopause and menopause. Birth control is the most common workaround, but it's far from the only one. First it's often therapy, an SSRI, meditation, CBT, lose weight, exercise more, sleep better, eat better, then 'just try birth control for 3 months.' None of it is HRT. You can refuse, and the scripts for doing it without conflict are in the course.
The dosing problem
This is where most menopausal women get stuck. Even once you're on HRT, system-wide policy keeps you at the minimum effective dose. Not fully functional, minimally functional. It traces to a 2002 study whose interpretation was re-examined in 2022, but prescribing practices haven't caught up.
The titration staircase
Lowest dose, wait 3 months, assess, repeat. The 3-month wait is a default, not a requirement. Your PCP can increase your dose; it doesn't have to go through OB/GYN. And some providers refuse to increase your dose at all, even in full menopause. The course teaches the levers that get them to move, and to advocate for the pace that's right for your body, faster or slower.
The estradiol-progesterone problem
Providers often increase estradiol without adjusting progesterone. If you have a uterus, these two often need to stay in balance (without a uterus, you may not need progesterone at all). The course gives you the exact question to ask every time your dose changes.
The moment of truth
