Medication & how-you-feel log
To bring to your prescriber, not to self-adjust
Read this first
This is a tracking sheet to share with the person who prescribes for you. It is not dosing advice. Never change, stop, or add a medication based on this page. Always talk to your prescriber.
The log

Questions to bring to your prescriber
"Is what I'm noticing expected, and does it usually settle?"
"How long before we'd know if this is the right fit?"
"What should make me call you sooner rather than wait?"
When to call, not wait
If you ever feel physically unwell, have chest symptoms, severe mood changes, or anything frightening, contact your prescriber or seek urgent care right away. Don't wait for the next scheduled visit. This log is for routine tracking only.
